Does Sperm DNA Quality Affect Fertilization Rates in IVF?

When trying to conceive through IVF, many couples worry about sperm quality, especially when the male partner is over 40. One common concern is DNA fragmentation which some believe could lower fertilization success. But is that really the case?

At Dunya IVF, our doctors Dr. Alper Eraslan and Dr. Berk Emin Angün, with the help of another doctor, conducted a study to see if DNA fragmentation index (DNA-FI) and high DNA stainability (HDS) affect fertilization rates in couples where the woman is under 35 and the male partner is over 40. Here’s what they discovered.

The Study – What Was Tested?

Between January 2022 and July 2024, our team analyzed 103 IVF/ICSI cycles at our clinic in Kyrenia, Northern Cyprus. The goal was simple: to see whether sperm DNA issues had any impact on fertilization rates.

Only couples where the woman had a good ovarian reserve were included.
Men with severe sperm issues or those using antioxidants were excluded.
Standard IVF protocols were followed, and sperm quality was measured, including:

  • Sperm count, movement, and shape (Kruger criteria)
  • DNA fragmentation index (DNA-FI)
  • High DNA stainability (HDS)

To assess the results, fertilization rates were grouped into:

  • Below 65% (low)
  • 65-80% (normal)
  • Above 80% (high)

The Results: 

  • 39% of men had poor sperm shape (teratospermia)
  • 31% had increased DNA fragmentation, while only 2% had high DNA stainability
  • Neither DNA fragmentation nor high DNA stainability impacted fertilization rates

What Does This Mean for IVF Patients?

For couples where the woman is under 35 and the male partner is over 40, sperm DNA fragmentation and high DNA stainability do not appear to reduce fertilization rates. While this is reassuring news, our team is continuing research to explore whether these factors affect pregnancy and live birth rates.
